Benefit Summary

Applicable benefits begin 1st of the month following one full month of employment.

 

Health Insurance

 •    Health Partners Open Access Plan  

 •    Core plan – Empower HSA -- $2,700 individual/$5,450 family deductible; MRCI contributes $650 individual/

      $1,300 family to the employee’s Health Savings Account.  Employee may contribute to Health Savings

       Account on a pre-tax basis.

 •     Buy up plan – NationalOne HRA -- $4,000 individual/$8,000 family deductible; MRCI’s commitment to pay

       $2,000 individual/$4,000 family of deductible expenses through Health Reimbursement Arrangement

 •     Wellness Plan – Employees may receive a monthly discount for participating in an online Health Assessment;

       $10 per month individual/$20 per month family

 

Dental Insurance

 •     Provided through Delta Dental

 •     $25 deductible; $1,000 calendar maximum benefit; 100% coverage preventative services 

 

Group Term Life Insurance

 •     Term life insurance provided through The Standard Insurance Company

 •     Benefit equal to 1 times salary and includes accidental death & dismemberment

 

Voluntary Term Life Insurance

 •     Optional coverage available to employees, spouse and dependents

 •     Policy available in increments of $10,000 up to $100,000 maximum benefit

 

Flexible Spending Account 

 •     Health care spending reimbursement account – ability to pre-tax medical expenses

 •     Health care spending reimbursement account limited to vision and dental expenses for those participating

       in Empower HSA Insurance Plan.

 •     Dependent care reimbursement account – set aside money pre-tax for dependent care expenses
